  Chronic Care Management

  What is Chronic Care Management? To answer this question, you must first understand why someone might need chronic care and how that differs from acute care. Let’s look at the definition of each: Acute care: “Acute care is a level of health care in which a patient is treated for a brief but severe episode of illness, for conditions that are the result of disease or trauma, and during recovery from surgery.” – From Connecticut’s Office of Health Care Access. Chronic care: “Chronic diseases are defined broadly as conditions that last 1 year or more and require ongoing medical attention or limit activities of daily living or both.” – From CDC National Center for Chronic Disease. When we think of hospital visits and treatment, those conditions putting patients in their care generally fall under the acute care definition. Here at Bates, Miller & Sims our doctors and other providers help their patients manage ongoing conditions that fall under chronic care.

  Kids Wellness Services

  Kids Wellness Service: Pediatric Care, School Physicals & Sports Physicals To fulfill the goals of providing care for a lifetime and for your whole family, the staff of Bates, Miller, & Sims includes kid wellness exams and physicals for both school enrollment and sports. Child wellness check-ups can be scheduled annually or situationally. Our pediatric doctors and staff are available for your children when they are sick, and when they are well. We can see your child immediately when you’re in crisis. However, our staff schedules well-child visits, vaccinations, and required physicals when needed.

Our affordable QuickCare (urgent care) services are an asset to the community we serve. Who Needs QuickCare (Urgent Care)? If you have an illness or injury that requires immediate medical attention but is not life-threatening and does not require a trip to the Emergency Room, our urgent care services are the perfect solution. Our practice offers both adult and pediatric urgent care services to the Stanford community. We welcome current patients and new patients in our QuickCare clinic. We are open at convenient hours for both appointments and walk-in patients to better serve you and your family
